Clean means to get rid of visible dirt or other contaminants. A good scrubbing does kill most germs, but not all. Getting rid of all the germs requires disinfection.

Disinfection Came Before Germ Theory
It makes sense that the big push for disinfectants came after the realization that there were germs to kill, but the idea of disinfection came earlier than that realization.
Ignaz Semmelweis, a physician in 1840s Vienna, was among the early proponents of antisepsis before the idea of sepsis arose. He and others (such as Florence Nightingale, a statistician, and a pioneer in nursing), he believed that dirt, blood, or something caused illness. He ordered students to wash their hands and disinfect them (in carbolic acid) before assisting women in births. The death rate dropped dramatically (to under 2%).
